IRIS in Action

Today, families experience stronger connections to services in 35 networks across 44 counties in six states.

As IRIS networks and participating organizations grow, so does their impact. Rising referrals and families served each year show sustained investment and increasingly coordinated pathways for families.

Every referral represents a provider taking a meaningful next step on behalf of a family. To date, communities have made more than <em>146,000 referrals</em> serving 80,463 families.

Families rarely face challenges alone. When organizations can refer across sectors—healthcare, housing, food access, behavioral health, education, transportation, and more—they’re better able to address the full range of factors shaping family stability.

All sectors are represented among the 2,700 programs participating in IRIS. This cross-sector referral capacity allows providers to move beyond addressing a single need, instead supporting the whole family.

Loop closure rates across most networks average 96, meaning the receiving program reviewed the referral, determined eligibility, conducted outreach to the family, and completed the referral process.

This high loop closure rate not only benefits families, it also builds the trust and accountability that strengthens system-level coordination.
