Over the years of leading IRIS for their community, Healthy Chelsea has built a strong foundation for coordinated care through strong partnerships, provider engagement, workforce training, and the expansion of a robust referral network. Their commitment has fostered stronger collaboration across organizations, improved access to resources, and created new opportunities to address community needs in a more connected and equitable way.

As the community marks this milestone, they are also excited to celebrate a new chapter. Leadership of IRIS is transitioning to the North Suffolk Public Health Collaborative, ensuring that the platform remains a sustainable and growing asset for the communities they serve.
